Olapex F.C. Tablets - Olanzapine - Antipsychotic agent.

 COMPOSITION

  • Olapex 5 mg: Olanzapine 5 mg.
  •  Olapex 10 mg: Olanzapine 10 mg.

PROPERTIES

Olanzapine has shown a broad pharmacological effect across a number of receptor systems. Olanzapine demonstrates greater affinity for serotonin receptors as well as dopamine receptors. 

Olanzapine is well absorbed after oral administration and reaches peak plasma concentration within 6 hours. The absorption is not affected by food. Olanzapine is metabolized in the liver and the mean terminal half-life is 30 hours.

INDICATIONS

  • Olanzapine is indicated for acute and maintenance treatment of short term schizophrenia and other psychosis where positive and / or negative symptoms are prominent. 
  • Olanzapine also alleviates the secondary disorders.

DOSAGE & ADMINISTRATION

The recommended daily dose is 10 mg once daily adjusted to usual range 5-20 mg daily. 

Doses of 15 mg or greater should be given only after reassessment. 

Starting dose of 5 mg per day is considered for geriatric patients and patients with severe renal or moderate hepatic impairment.

CONTRAINDICATIONS

  • Patients with known hypersensitivity to olanzapine.
  • Angle closure glaucoma. Breast-feeding.
  • Pregnancy use only if potential benefit outweighs risk.

WARNINGS AND PRECAUTIONS

In neuroleptic malignant syndrome and tardive dyskinesia, discontinuation of all antipsychotic drugs including olanzapine is required.

Use in specific population: Pregnancy (Non-teratogenic effects): This product should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.

withdrawal symptoms following delivery. There have been reports of agitation, hypertonia, hypotonia, tremor, somnolence.

Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for extrapyramidal and/or respiratory distress and feeding disorder in these neonates. These complications have varied in severity, while in some cases symptoms have been self-limited; in other cases neonates have required intensive care unit support and prolonged hospitalization.

Olanzapine should be used with caution in patients with:

  • Hepatic impairment. 
  • Anticholinergic drugs.
  • Centrally acting drugs.
  • Alcohol.
  • Prostatic hypertrophy.
  • Paralytic ileus.
  •  Low leukocyte or neutrophil count.
  • Bone marrow depression.
  • Hypereosinophilic disorders
  • Myeloproliferative disease.
  • Concomitant administration of drugs that prolong QT interval (especially in elderly).
Increase chance of death in elderly patient with dementia-related.

SIDE EFFECTS

Weight gain, somnolence, dizziness, akathisia, constipation, dry mouth, orthostatic hypotension and peripheral edema are occasionally seen with olanzapine, blood dyscrasia, transient asymptomatic elevations of liver enzymes.
